Before Submitting Your Request
Please keep the following in mind:
Plan ahead. Submit your refill request at least 3–5 business days before you expect to run out of medication.
Allow time for processing. Refill requests are reviewed during regular business hours. Additional time may be needed if an appointment, lab work, or provider review is required.
Check with your pharmacy first. Your pharmacy may already have refills available on your prescription.
Keep appointments up to date. Some medications require periodic follow-up appointments or monitoring before a refill can be authorized.
Controlled medications. Requests for controlled substances may be subject to additional requirements and cannot be guaranteed through this form.
Important Information
Submitting a refill request does not guarantee that a prescription will be approved or sent to your pharmacy. Your provider will review your request and determine whether a refill is appropriate.
Please do not use this form for urgent medical concerns or emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency department.
For questions about your medication or a time-sensitive concern, please contact our office directly.
